Managing Chronic Diseases
The practice provides medical yearly reviews for patients who have chronic diseases, also termed as Long-Term Conditions which are listed below. Depending on a patient’s medical condition, the reviews may be more regular. The review then can be 3 monthly or 6 monthly.
Our Nursing Team conduct these reviews and liaise with the GP who, if required will be involved in the care of the patient.
- Asthma
- Diabetes
- Hypertension
- Chronic Ophthalmic Pulmonary Diseases (COPD)
RECALLS
The practice runs a recall system whereby patients due for reviews are contacted to make an appointment with a clinician and depending on the nature of the medical condition; the appointment may be with a GP, Nurse or Health Care Assistant.
It is IMPORTANT for patients to respond to these invites as it has a bearing on the patient’s health and medical care as well the repeat prescribing of medication.
The practice appointment methods are by telephone and video. Face to Face appointments are available if the clinician determines that it is necessary.
