Update your records

Patients are required to update their records, if they move address, change their mobile number or email address.

If records are not updated, this may delay referrals and delay the practice from providing you with the medical reviews that your health or medical condition may require.

Any change of name must be accompanied by the original document which confirms the name change.

Please note, the practice will contact you for de-registration if you new address is outside our catchment area
